We provide case management services. We will
work to create a written process to enhance the consumer’s
functioning and/or integration into their community by establishing
a relationship with the consumer, or their family, or their legal
representative(s). The Consumer’s appropriate services and/or
benefits will be established, planned, obtained, provided, recorded,
monitored, or modified when it is necessary and/or this service
is terminated. We might provide an assessment to determine the
Consumer’s needs (basic or otherwise), assist them in obtaining
entitlements, or assist them in finding necessary resources along
with their covered services. |
