When we receive your request for Case Management,
we will make immediate contact with the Consumer/Family/ or Consumer's
Legal Representative in order to assess the referred consumer for
appropriate services. It will be decided at that time who will
be the responsible entity to conduct the initial intake of the
consumer and/or whether this agency will only be performing the
proposed ISP and continual support to the consumer. Within 10 days
of your referral, we will inform the referral source either in
writing or verbally whether we will serve this consumer. Once
this agency receives the service authorization from the referring
source, and the consumer is assigned to a service operated by,
or financially supported by, the referring source, an annual ISP
will be developed after this official authorization. We will submit
the proposed ISP to the referring source for review and authorization. |
