When we receive a request from you to assist
with developing a plan for Person Centered Planning (PCP), we will
need to know whether we will be conducting the initial inquiry
of the individual or whether we will only be facilitating the proposed
PCP.
PCP is a process that refers to the facilitation and development
of a plan that establishes a relationship with an individual and
their family, their legal representative, and/or other important
people to the individual. We will contact and meet with the individual,
and the other important people to that individual, to explain the
PCP process and to determine others that the Individual would like
to have to participate in their own plan. We would work with
the individual and any legal representatives to determine a time
and location for the PCP that will assure the Individual’s
participation as well as others to be in attendance.
We will develop a plan to enhance the individual's functioning
and/or integration into the community incorporating the following
objectives:
We will:
- facilitate the planning session,
- assist the Individual to participate as much as possible,
- establish ground rules,
- be positive and keep the group positive and focused on the
individual’s choices and strengths,
- record the Individual’s vision of the future,
- gather and organize information,
- respect the person’s choices and preferences,
- focus on capacities of both the person and the community in
which he or she lives,
- provide an accurate picture of the person and their desires,
- be action oriented with action steps
- establish timeframes for evaluation performed and followed
through upon.
- We will provide follow up on action steps by bringing the group
back together within 3 months of the initial plan,
- then provide follow up on action steps whenever it is requested
to help to provide ideas and recommendations for future follow
up meetings.
The plan will focus on both paid and natural supports to assist
a person in achieving their desired future. We will write
the plan up and provide a copy of the plan to the Individual and
any legal representative for them. |
